New Patients

New Patient Registration

We are pleased to welcome new patients who live in the catchment area. To register at the practice you will need to complete a registration form for each person. Forms are available at reception. We invite all new patients to make an appointment with the practice nurse on registration. At this appointment the nurse will check your blood pressure and urine. You will also be asked about your medical history and medication, and you will be offered advice related to your general health. 

Medical treatment is available from the date of registration. Please contact reception for further information.

Online “Pre-Registration” with the Practice

If you wish to pre-register click on the links below to open the forms. When you have completed all of the details, click on the “Send” button to mail your forms to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Practice Area

Patients are accepted who reside within the area bordered in black. The practice serves the areas of Altofts, Kirkthorpe, Normanton and Sharlston Common and parts of Stanley and Whitwood. If a current patient moves to the area bordered in red then they can stay registered with us.  Unfortunately we are unable to accept new patients from this area.
